By submitting this form I explicitly confirm that:

1. I understand that Eastwright Corp only provide software services on Unix (Apache) and NT/W2K (Apache or IIS) servers.

2. I understand that fees are $39.95 per 1/2 hour with a 1 hour minimum charge. My credit card will be charged for $79.90 prior to beginning the installation. Most installations require only 1 hour of work, however some may require more. Each server is different. If Eastwright Corp need to go over 2 hours of work, I will be contacted before installation continue. Once the installation is completed, my credit card will be charged for additional fees if any.

3. I understand that Eastwright Corp is not responsible for the hosting provider I have chosen. While every effort will be made to set up software, no guarantees have been made that the software will be able to function on my server. I also understand that I am paying for the installers time only and I am still responsible for all charges regardless of the outcome of the installation.

4. I understand that I agree to provide Eastwright Corp with temporary ftp account on my server. The directory scope and access permissions of this account must be sufficient to complete the works ordered.

5. I understand that I agree to provide Eastwright Corp with all information required to complete the works ordered.

6. I understand that Eastwright Corp begin installation within 24 hours (Mo. - Fr. excluding public holidays) of receipt all applicable payments.
